Follow These Easy Steps to Start Earning Your Rewards

Step 1: Complete the online Health Risk Assessment

• This is located on UHC Rewards.
• Confirm you have a PCP while taking the Health Rish Assessment

Step 2: Complete Age Recommended Screenings

• If you are due and your doctor recommends it complete any of the age or gender preventive screenings.            

Step 3: Complete an Annual Eye Exam

Completion of step 1: Reward is PCP copay waiver

Completion of step 2: Reward is $5 reduction on your specialist copays

Completion of step 3: Reward is $5 reduction on your specialist copays

Completion of steps 1,2 & 3: Reward is PCP copay waiver and $10 reduction on your specialist copay

Once all steps have been completed the member will have:

• $0 PCP Copayment
• $20 Specialist Copayment

** The plan reset every two years (on the odd year). CY2027 members will start fresh with no reduced copayments.
